What is the main point of lengthening and widening of the penis? How are the procedures done?
Phalloplasty is a procedure aiming to lengthen or widen the penis.
The main point of phalloplasty (lengthening of the penis): the ligament located atthe pubic bone and supporting the base of the penis is cut. Such procedure and wearing of an extender for 2 months after surgery lengthen the penis by 3–4 cm when flaccid. It can help men feel more confident, open some new sexual experience.
Sometimes, after lengthening of the penis, patients wish to widen it, as well. This wish can also be fulfilled. Plastic surgeon Pavel Denishchuk performs procedures for increasing the diameter of the penis by injection of patients’ own adipose cells (lipofilling). Indications

Contraindications
Phalloplasty is not performed in patients with:
● infectious diseases of the genitourinary system;
● recent history of stroke or heart attack;
● blood coagulation disorders;
● syphilis, hepatitis, or HIV;
● benign or malignant neoplasms of the organ.
Also, the procedure is not performed in men under 18 years of age.
Special aspects of the recovery period
Immediately after the procedure, men may experience aching sensations, swelling. Typically, they resolve within 2 days after the procedure. In a week, the suture after phalloplasty is completely healed.
To achieve the maximum effect after the procedure, men have to wear an extender for at least a month. Such device, worn on the head of the penis, creates elongation, which stretches the ligaments, extending the penis by a few extra centimetres.
To prevent complications, surgeons advise avoiding tight underwear, active sports for 4 weeks. It is also recommended to avoid intercourses for this period of time.
To prevent suppuration and necrosis, the patient shall take care of a postoperative suture, protect it from getting wet and friction. Therefore, physical activity and going to swimming pools are extremely undesirable.
Also, avoiding intercourses for up to 1 month is required after lipofilling of the penis, otherwise the fat may not take.
What results can be achieved with the surgical procedure
After phalloplasty, patient’s penis may lengthen up to 4 cm, after lipofilling — widen in diameter by 1–1.5 cm.
In order for a man to have an accurate idea ofthe result, surgeons explain that the effect of the procedure depends on:
● Anatomy — sometimes in men the scrotum slides onto the base of the phallus, which visually shortens it. Therefore, for a pronounced effect, it will be necessary not only to cut the ligament, but also to adjust the location of the scrotum.
● Endocrine profile.
Possible complications
Penile enlargement may be accompanied by the following complications:
● Scarring. The degree of scarring depends on the individual characteristics of the body. In some patients, the incision site is generally discreet; in other men, a scar develops after the penis lengthening surgery.
● Erectile dysfunction. When a penis undergoes a lengthening procedure, no nerve fibres are damaged, so there should be no problems with erection. Such disorder is purely psychological in nature. Therefore, such complication can be addressed in psychotherapy sessions.
